Before receiving an assignment, volunteers must complete three forms: a Volunteer Application, a Volunteer Survey, and a Missouri Request for Child Abuse or Neglect/Criminal Record form. Columbia Public Schools' Community Relations Department is responsible for sending this to the state for review. Volunteers do not pay any fees to have background checks processed.  All information is strictly confidential. Once all paperwork has been processed, volunteers will be contacted regarding their training and volunteer assignment.
